Mistakes and fixes
Preserve the original decision, the proposed remedy, the approval and the verification as separate records.
Preserve the original decision, the proposed remedy, the approval and the verification as separate records. Do not erase an earlier decision because a later one replaces it. Keep both, and say which one is current.
Observation
What was expected, what the record actually shows, and what is still unknown. Do not assign blame from an incomplete record.
Proposed remedy
A suggestion for what to change. A suggestion is not the change.
Approval
A person agrees to that remedy, or does not. The original decision stays visible either way.
Verification
A later check of what changed. Until that check exists, the remedy is not verified.
